Designing a Rights-Based Protocol to Reduce Child Malnutrition in Hormozgan Province

چکیده :
Abstract Background and Objective: Child malnutrition is one of the most serious public health challenges, with extensive physical, cognitive, social, and economic consequences. In Hormozgan Province, as one of Iran’s deprived regions, the prevalence of severe stunting and underweight among children under five years old is reported to be higher than the national average. This study aimed to design and develop a localized, rights-based protocol to reduce at least 30% of severe stunting prevalence among children aged 6 months to 5 years. Methods: A participatory approach and situation analysis were employed in this study. Following the needs assessment, stakeholder analysis, and evaluation of stakeholders’ power and interest in participating in the program design and implementation, general and specific objectives were developed using the SMART model. Based on scientific evidence and stakeholder prioritization, executive strategies were formulated, and operational tables, Gantt charts, cost estimations, and an evaluation plan were developed. Findings: National and international studies demonstrated that the use of nutritional supplements (such as RUTF, SQ-LNS, and CSB+), alongside educational and food support interventions (such as food baskets and nutritional vouchers), significantly improved anthropometric indicators and reduced the risk of mortality among children. Within this protocol, food baskets and therapeutic nutritional supplements were designed for the lowest three economic deciles in the province, accompanied by strategies for identifying eligible households, implementing the interventions, providing education, and monitoring effectiveness. Conclusion: The design of this protocol, based on scientific evidence and children's rights principles, and with stakeholder participation, presents an executable and scalable model to reduce child malnutrition in deprived areas. Sustained financial support, continuous education, and performance monitoring are crucial for the success of similar programs nationwide.
